Whether you're living with a condition or simply wanting to stay on top of your condition, British Heart Foundation (BHF) has the tools and support to help you feel confident about your heart health. The NHS Healthy Choices Quiz
This initiative by the NHS aims to help individuals to review how healthy their lifestyle is. The quiz takes about 5 minutes and reviews 6 areas of an individual's lifestyle. It provides a summary of each area and recommends which Apps and advice you will find helpful in order to make health choices that last, similar to those shown below.
The picture below shows the areas the quiz covers. Click on the blue info box below to start the quiz.
The information is not shared with others and stays on your device that you use. The quiz is for adults aged 18 and over. You should not use the quiz if you are 17 or under or have existing health conditions such as diabetes or heart disease.

The internet provides a wealth of information about practical ways you are able to maintain both your physical and mental health & wellbeing. The Patient Champions are offering a range of activities to help improve the ability of patients to take more responsibility for maintaining their personal health and wellbeing through practical activities.​

NHS App
You could take more control of how you manage your health & wellbeing through using the NHS App. Some of the things you could do are shown in the image above.

Patient Access App
Patient Access lets you use the online services of the practice. These may include arranging appointments or repeat medication, viewing your medical record and updating your details.

You will need to get a user ID from the Practice to be able to register for this service.

There is a lot of information available online to allow you to understand how to maintain your health and wellbeing. There is a team of patient champions able to guide you through the internet maze to find what will be of most help, either individually or in group sessions to promote the use of the NHS and Patient Access Apps. These allow you to contact your GP surgery to order repeat prescriptions & make appointments.
